About MHT in Menopause

Progesterone is prescribed for women with a uterus to protect the uterine lining from the effects of estrogen. Cyclic use (12–14 days/month) can reduce spotting; daily (continuous) use eliminates cycling altogether. Progesterone is best taken at bedtime — its metabolites have a natural sedative effect.

Estrogen can be delivered via transdermal patch or daily (oral, gel, or spray). Patches deliver bioidentical estradiol transdermally, bypassing the liver. Weekly patches (Climara, Menostar) are changed on the same day each week. Twice-weekly patches (Vivelle-Dot, Minivelle, Dotti) are changed on two consistent days per week, ideally 3–4 days apart (e.g. Mon & Thu). Daily estrogen (oral estradiol, transdermal gel, or spray) provides steady estradiol levels with once-daily application.
